How to Become an MHMC Volunteer
Step 1 — Complete the Volunteer Application
Download, print, and complete the MHMC Volunteer Application.
Step 2 — Complete the HIPAA Compliance Agreement
Download, review, and complete the MHMC HIPAA Compliance Agreement. Protecting patient privacy and confidential information is an important responsibility of every volunteer.
Step 3 — Bring Your Completed Documents to MHMC
Please bring your completed Volunteer Application and HIPAA Compliance Agreement to the clinic during a scheduled clinic session.
We recommend contacting MHMC before your visit to confirm volunteer needs and arrangements.
